What's the difference between a clean ISP and a clean IP address?

Can somebody tell me if websites are banning the ISP of the hosting server or the IP itself? Or perhaps both?

The IP address may vary regularly after the lease expires, depending on your ISP. The most common cause of a rapid change in IP address is a disruption in the connection between your router and the internet. Your ISP will assign a new IP address when you rejoin to the internet.
IP address refers for Internet Protocol Address, while ISP stands for Internet Service Provider. Your IP address, on the other hand, notifies another computer which computer is attempting to interact with it and which one is making a request for information. All of this is now accomplished via ports and established protocols.
